Privacy and data protection
I hold patient contact details for the purposes of communication. I also collect certain other details such as those for a GP and any medical conditions to inform therapeutic work and for multi-disciplinary communication as and when necessary. I hold phone numbers on a separate work phone under initials only and the device is password protected.
I keep notes in order to reflect on and track any ongoing work. I may use information from these notes to discuss in 1:1 supervision. These notes are anonymised and I do not use full names nor share these with anyone else. They are stored in a locked filing cabinet and shredded 5 years after ending therapeutic work.
Patients have a right to request a copy of the information I hold about them, which I can provide with a month’s notice, and they can withdraw this consent in terms of holding information at any time.
